Purpose: Create a Reports Page from the Perspective of a Patient

Sallie should be on the page showing patients where they have improved.
Second
Patients know what bothers them the most, but it is helpful to visually see it in a list.

Providers still need the ability to see a more complex display of data.
Fourth
The charts should have fewer words to reduce visual strain.
